| To Register for Tri-City Youth Ice Hockey Association: 1. Access and complete the USA Hockey online registration form at www.USAHockey.com. ALL PLAYERS must register with USA Hockey prior to being placed on a team roster. Registration with USA Hockey enables proper eligibility for USA Hockey roster placement (required by the league we play in and tournaments you might attend) and insurance coverage for all players at the beginning of the 07/08 season. 2. When you complete your USA Hockey online registration, you will receive an email with your confirmation coade that looks like this. Please forward this email to Kelly Peterson or print the confirmation page and mail to address listed below. Kelly will use this to place you on a team roster. She cannot place you on a roster without this confirmation code. Please direct any questions to Kelly Peterson problems processing your registration or if you are unable to use the USA Hockey online system for any reason. 3. We do offer financial scholarships to players who qualify based on financial need. To apply, complete the Scholarship Form and the scholarship committee will review the request and let you know of your status. 4. Anyone wishing to skate up must also complete the Request to Skate Up form and mail with application 5.
